For doctors who agree to practice full time in a federally designated shortage area, or at a Veterans Affairs facility, for a total of five years. It needs a letter from a federal agency or state health department confirming the work is in the public interest.

For physicians whose clinical, research or public health work is of national importance, wherever they practice. It uses the same three-part test as other NIW petitions and carries no five-year service rule.

Does my J-1 waiver service count?
J-1 면제 근무 기간도 인정되나요?
Often yes. Time spent working under a J-1 waiver (for example, Conrad 30) can count toward the five years, within the rules. We check how your dates line up.

Under the standard NIW, yes, as long as you continue the proposed work. Under the physician NIW, changes need care to keep the five-year requirement on track.
